Groups

  • Add or remove providers
  • Add or remove locations
  • Change an address (billing and/or mailing)
  • Update practice information (hours, services, age limitations)

You can use this form to update TIN/group information i.e, one or more locations, and for specific providers or all providers at a location.

Individual Providers

  • Change provider name
  • Add languages
  • Change an address (office and/or billing)

You can use this form to make these changes for a single provider NPI with one or more locations. Changing an office address allows you to update the street address, suite, phone, fax and email.
