Who needs to enroll in New Hampshire Medicaid?
Any provider who would like to receive Medicaid reimbursement for services rendered to Medicaid recipients. This includes rendering and billing providers, groups, and facilities.
How do I enroll?
Providers can review enrollment instructions by visiting the New Hampshire Medicaid website. To complete the registration process, select Enrollment, then select the type of enrollment to complete the appropriate online application.
Per the Provider Enrollment FAQs, NH Medicaid offers four types of enrollment applications for providers:
- Group Provider Enrollment
- Individual Billing Provider Enrollment
- Non-Billing Rendering Provider Enrollment

To track the status of your enrollment, go to Provider Enrollment, locate Application Status, then enter the Application Tracking Number and click Submit.
